About The Service

The Inclusion Team work with, support and coordinate services for children with additional needs and disabilities, their families and settings/schools to ensure that children access their entitlement with smooth transitions into a range of settings. We work in partnership with parents, education services, and other professionals in education and health.

The Inclusion Team monitors and supports schools/settings to successfully include all children with additional needs and disabilities to access their entitlement, environment and make progress.

 

About the job

An exciting opportunity has arisen for a Transition Worker, working as part of the wider Inclusion Team within the Education Service and with partners in many different agencies.

The role focuses on working with parents/carers ensuring that the needs of children with additional needs and disabilities are supported to transition to different settings, access their entitlement and make good progress.

We are looking for a highly organised, motivated and passionate individual who can join our dynamic forward thinking team to improve outcomes for children through partnership working.
