About The Service

An exciting opportunity has arisen within Independent Living Services, as a Hospital Contact Officer, for the First Point of Contact (FPOC) Hospital Team.

Independent Living Services (ILS) support vulnerable adults to live independently at home and connected to their communities, through tailored information, advice and support – enabling people to take control of their own lives.

The FPOC Hospital team (also known as the “Pink Army”), are a fundamental part of the University Health Board (UHB)’s integrated discharge team, providing a person centred approach to the quick and safe facilitation of discharge for Cardiff patients. Our team also acts as a conduit to all community services and point of communication for many partners.

Currently the FPOC Hospital team are based in the University Hospital of Wales, Lakeside Wing, and Llandough Hospital, with the possibility of expansion to further UHB sites.

About the job

The successful candidate will be responsible for working as a member of the FPOC team; providing advice, information and assistance to members of the public, both in respect of services directly provided by Adult Services and those provided by other relevant agencies.

The contact officer will also help support to co-ordinate hospital discharges, to promote excellent standards of discharge practices across the University Health board (UHB) sites to improve the quality of the patient experience. This will include the collection and review of appropriate data to measure the impact of FPOC team practices as part of the UHB integrated discharge team.

What We Are Looking For From You

The successful candidate will have excellent communications and IT skills and experience of working within a Social Care, Health, 3rd Sector or Advice-giving environment. You will have a strong track record of managing resources effectively and providing excellent outcomes for customers.

You will need to demonstrate an ability to respond sensitively to individual’s circumstances, as well as having a positive can-do attitude.

This post is temporary until 31st March 2023.
