About The Service

An exciting opportunity has arisen within Independent Living and community social care, as a higher clerical assistant, for the Integrated Discharge Hub.

The Integrated discharge Hub (IDH) is made up of a multidisciplinary team that comprises of colleagues from clinical, social care, preventative and holistic specialties. The IDH is responsible for the triage of patients, providing a person-centred approach to the quick and safe facilitation of discharge for Cardiff patients. Our team also acts as a conduit to all community services and point of communication for many partners. The IDH team operate their services across all University health board hospital sites.

About the job

The successful candidate will be responsible for working as a member of the Integrated discharge hub (IDH) team; providing information to members of the public and colleagues, both in respect of services directly provided by Independent Living Services and those provided by other relevant agencies. You will also contribute to the provision of an effective business support service within the Integrated Discharge Hub, maintaining support systems in respect of, referrals, telephone messages, databases, registers, correspondence, minute taking, photocopying, filing and stationery supplies.

What We Are Looking For From You

The successful candidate will have excellent communications, IT skills and experience of working directly with members of the public/other agency personnel. Experience of working within a Social Care, Health, 3rd Sector environment is desirable. The candidate will have a strong track record of dealing with customer enquiries and complaints diplomatically and adeptly. As well as managing resources effectively and providing excellent outcomes for customers.

You will need to demonstrate the ability to prioritise and work within a high-pressure environment, work autonomously and as part of a team and excellent organisation skills.

The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.
