About The Service

The Community Resource Team (CRT) is an integrated Health and Local authority service run by Cardiff Council Independent Living service and Cardiff and Vale UHB. Our team is based across the city and consists of various social care and health professionals.

The CRT provides help to adults (over the age of 18) living in Cardiff who need support to stay independent, within their own homes.

About the job

Home Care staff will work in partnership with the client to achieve their goals and improve their independence.

A service delivery plan supports the carer in their daily duties. Carers work to a rota, working AM and PM shifts. Each carer has an electronic device that allocates their daily tasks. This is an important role working to a structured plan and specific goals. The need for good observation and recording skills are essential, a flexible, focused approach is essential

These are temporary posts (initially until 31.03.2022 with the possibility of extension) available.

Internal candidates who wish to apply for this position on a secondment basis must obtain approval prior to application using Form SEC1 (4.C.081). Requests may only be approved by the relevant Director/ Assistant Director/ Chief Officer or senior nominated officer graded no lower than OM2 or in the case of schools based staff the Headteacher / Governing Body.

Contracted hours will be 25 hours per week.
