About The Service

Our Service offers primary community-based support for older people and people with a physical disability. We are a team comprising of Social Workers, Social Work Assistants and Home Support Workers who together, offer support to citizens aged 18 to 100 years plus.

About the job

We are seeking to recruit Social Worker Assistants to join us in the Case Management and Review Team.

We work alongside our colleagues within Adult Services, we aim to move people seamlessly through a pathway in identifying the support they require. Within the case management and review team functions, we support complex cases and arrange a varied range of support from community-based packages of care, care home permanent placements, shorter-term respite placements and hospital discharges. We work in conjunction with a range of professionals that include our health colleagues, occupational therapist and secondary mental health teams in assessing and reviewing those citizens who have an eligible need for care and support under the Social Services and Well-being (Wales) Act 2014.

Our philosophy is to foster a culture where employees are valued, supported and encouraged to reach their full potential. We provide staff with regular supervision alongside a informal approach for guidance and support as required. We offer and encourage relevant training sessions. We hold a weekly team meeting to discuss and share information relevant to our social work-based roles. We are based in County Hall, Atlantic Wharf, Cardiff Bay, but also work from home.

The team work closely with citizens, families, and carers to ensure they are safe and live well in their communities.
