This post is an exciting opportunity to be part of the delivery of services for young people with care and support needs accessing Homelessness Services. The post is managed by the Adult Services Team Manager responsible for Transitions for Young People who are Looked After and will work alongside colleagues in both Adult Services, Specialist Support Services Team and the Homelessness Multi-Disciplinary Team. About the job

The role requires that you are a Registered Social Worker with Social Care Wales. You will be undertaking assessments with young people with care and support needs moving into adult social care and Homelessness provision. This will require close multi- agency and multi-professional working with colleagues from Adult Services, Children’s Services and Homelessness as well as other agencies and colleagues. You will be required to commission outcome-focussed care and support packages and undertake direct work with individuals.

You will be required to work in a strengths-based manner with young adults supporting them to maximise their independence. You will be required to work within the Social Services and WellBeing Act (Wales) 2014 Legislation and the Mental Capacity Act 2005. You will be required to prepare for and attend meetings where decision-making is required.

What We Are Looking For From You

• Experience of working with children or young adults with complex needs.
• Commitment to excellent practice with individuals, families, Carers and Communities.
• Assessment skills and a clear understanding of the Social Services and Wellbeing Act (Wales) 2014, Mental Capacity Act 2005 Legislation and Wales Safeguarding Procedures.
• Excellent communication skills both written, verbal and non-verbal and the ability to engage positively with young people with care and support needs.
• A commitment to partnership working and multi-professional working to promote good outcomes for all young adults.
• A commitment to working in a Strengths-Based way with individuals.
• Commitment to safeguarding and promoting the wellbeing of citizens is an expectation of all who work in the service area.
• The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.

Additional information

This role currently attracts a market supplement of £3,000 (full time equivalent), reviewed annually.

This post has a requirement for registration with Social Care Wales.
