About The Service
Our Support Planning team are part of the Directorate of Adults, Housing & Communities in Cardiff Council.
As a Directorate, we recognise that support planners are skilled professionals who make positive contributions to people’s lives every day. Working with two learning disability social work teams that cover the east and west of Cardiff. Our staff work alongside a specialist health team and a wide range of community organisations. We support individuals to access a wide range of opportunities with a focus on inclusion and supporting people to meet their outcomes. This is an exciting time to join a service that actively promotes best practice and supports strength-based working with its practitioners. An independent audit in March 2020 advised us we have areas of national good practice and we are keen to build on these and develop further high quality local opportunities to meet people’s outcomes as we recover from the pandemic.

About the job
Support Planner Post (1x 24hrs. part time) is available in the Learning Disability Adult Community Team.

What We Are Looking For From You
Dynamic and motivated individual to work alongside a well-established and dedicated team of Support Planners and Social workers.
We are looking to recruit a person who are passionate about working alongside Citizens with a Learning disability, their families and the community to bring about change and promote positive outcomes for each individual we support. You will need to have a strong value base, previous experience with working with people with a learning disability, strong organisational and time management skills, and have a commitment to strength-based practise and person-centred working initiatives.
You will be required to
• Design high-quality, person-centred plans that represent the citizen and their chosen lifestyle.
• Work to a clear value base that promotes self-determination and social inclusion within the citizens local community and support the citizen to establish and maintain community connections.
• Support to empower citizens who use our services to reach their full potential.
• Signpost and provide accessible information on a wide range of opportunities available.
• Liaise efficiently with a wide range of internal and external partners.
• Establish and maintain a wide range of community based integrated opportunities for Citizens.
• Monitor and review referrals to ensure they remain person centred and that agreed outcomes are met.
• Maintain appropriate records and compile reports in accordance with any format required.
• Commitment to safeguarding and promoting the wellbeing of citizens is an expectation of all who work in the service area.
• Although not essential, the team would welcome Welsh speaking applicants.
