A vacancy has arisen in the Housing Support Team for a Health and Social Care Officer. To be responsible for the day to day operational aspects of the Cardiff Floating Support Service.

 

About the job

  • To provide advice and assistance to individuals and agencies about the Cardiff Floating Support Service.
  • To receive and assess referrals to the Cardiff Floating Support Service ensuring referrals to the scheme are effectively processed.
  • To enter details of referrals received on to the Cardiff Floating Support management systems.
  • To allocate referrals to service providers to carry out support needs assessments and ensure the assessments are carried out within the set timescales.
  • To allocate cases assessed to need support to support providers and monitor the progress of cases, reviewing them at set timescales.
  • To liaise regularly with support providers regarding individual cases and relevant aspects of the service.
  • To monitor the capacity of the service maximising the use of support spaces, liaising with providers to effectively manage individual cases who are not engaging with the planned support

What We Are Looking For From You

The post holder will take a lead role in allocation of services, working closely with support providers and service users to gather relevant information.

An understanding of the Housing Related Support is essential, as is the ability to communicate well with a wide range of stakeholders.
