About The Service

Independent Living Services (ILS) work in partnership with vulnerable adults to support them to live independently at home and connected to their communities, through tailored information, advice and assistance – enabling people to take control of their own lives.

The Independent Living and Well-being team are key to providing assistance to vulnerable adults in meeting their well-being goals.

About the job

The post will entail the oversight and co-ordination of the delivery of the Independent Living and Well-being Service, which incorporates, The First point of contact for both hospital and community, The Independent Living and well-being visiting officer’s, older peoples floating support and the Day Opportunities community engagement team.

The role will be responsible for delivering a person centred service which focuses on putting what matters to the people we support at the centre of decisions made, as well as performance and milestones, providing a co-ordinated response to customer’s and working with internal and external partners to improve service provision.

What We Are Looking For From You

You will need to be an extremely motivated individual with a passion for promoting independence through a strength based approach, as well as an excellent communicator with the ability to listen, act and inform.
