Job Purpose:
The Operational Manager for Reablement Services will be responsible for:
The strategic development and delivery of mulitple teams, whose key driver is to support people to stay at home, connected to their communities and empower independence.
The role will be the strategic lead for a number of services that support the preventative agenda: Community Resource Team, offering short term home care to enable discharge, or to avoid admission to hospital; the delivery of Disabled Facilities and Joint Equipment Services and the delivery of the Community Occupational Therapy Service. Ensuring care and support is provided at the right time and in the right place, incorporating reablement through direct care, blended with holistic community support, ensuring the “what matters” conversation is at the core of service provison.
Overseeing strategic policy, and procedural development across the services, ensuring staff are supported to deliver the services and policies effectively.
Driving forward a strengths based approach, developing cohesive integrated relationships with both statutory and independent agencies, inclusive of acute and primary care, care providers, 3rd and private sector. To ensure an effective integrated multi-agency approach to delivering local community and wellbeing services.
Provide regular evidential reports through detailed and expert analysis, on best practice. Using wide ranging evidence-based intelligence, with recommendations for necessary corrective action to influence future service delivery, demographic and financial challenges.
What We Are Looking For From You
• You must be a dynamic individual with a passion for promoting independence. You must be analytical, proactive, solutions focussed, with a commitment to continuous service development.
• You must also be able to assess risk and develop pre-emptive strategies for mitigating risk. You must be able to create and deliver effective partnership working with focussed outcomes and be able to engage, support, challenge and deliver change. You will be confident and flexible in your approach.
• You will be able to demonstrate in-depth knowledge of a wide range of preventative services to help people remain independent in their own homes and to prevent or reduce the need for care.
• You will have a good understanding of the Social Services and Well - Being (Wales) Act 2014 as it relates to prevention services, and regulated home care services.
• You will have extensive experience of working in a community, housing, and / or social care environment.
• You will be able to demonstrate experience of joint working across health and social care, private and third sector.
• You will be able to demonstrate experience of developing and delivering person-centred services working in a collaborative way that optimises independence and wellbeing.
• You will have proven ability to effectively manage and direct “resources” including employees, property, and technology to deliver projects and to meet set targets.
• You will have the ability to analyse business activity and develop clear evidence-based proposals for change.
• You will be able to demonstrate proven ability to lead and manage change: to plan and implement opportunities for service development and improvement.
• You will be able to demonstrate clear leadership skills, supporting and encouraging others to achieve good performance.
• You will demonstrate the ability to communicate to high standards in a manner appropriate to individual needs, by phone or in person or in writing.

• You will be able to demonstrates a positive attitude to new challenges and a willingness to adapt to quickly to change.
