About The Service

An exciting opportunity has arisen for committed individuals to join The Community Resource Team (CRT) Homecare which operates throughout Cardiff. We support the citizens of Cardiff preventing unnecessary hospital admissions, facilitating discharges from hospital and aim to assist people to live independently in their own home. We work closely with our health colleagues to support the service user with their reablement goals and to achieve what matters to them. We carry out an assessment with the service user in order to build a plan to support their individual needs and assist them to achieve their wellbeing goals.

About the job

Care staff will receive the recently regraded increased pay scale. Care staff will support the client to achieve their goals, working alongside health professionals. A service delivery plan supports the carer in their daily duties. Carers work to a rota, working shifts. Each carer has a mobile device that allocates their daily tasks. We are recruiting to more than one position within the team so various rotas and hours are available which can include evening and weekend work.

What We Are Looking For From You

The need for good observation and recording skills are essential, a flexible, focused approach is required to be able to work in a dynamic and fast changing service. A caring nature and the ability to train to achieve the relevant HSc (previously NVQ/QCF) level required for the post.
